FALL ATHLETES & BAND

Must register Parent Genesis (rollover around July 8th )»login»forms»Fall sport/band
The Physical must be current (w/in 365 days) of the fall sport start date or band camp (July 27, 2024)
Physicals performed prior to July 1, 2024 may submit the old Pre participation physical form. 
Physicals performed after July 1, 2024- SUBMIT NEW MEDICAL ELIGIBILITY FORM-completed by your physician-The medical eligibility form is the only form that should be submitted to the school. 
Health History Update-required for ALL despite physical date-(on Genesis)
Post Covid Clearance-required after recovering from covid
When a student indicates an allergy or asthma-an Action Plan is required for school/sport clearance
Paper forms only-(there will be a box in the main office during summer hours)

Athletes should check their eligibility status on Genesis




🤢 Little reminders regarding illness:


  • Stay home when sick
  • People who have symptoms of respiratory or gastrointestinal infections, such as cough, fever, sore throat, vomiting, or diarrhea, should stay home and talk to a healthcare provider for further guidance
  • Students and staff should consult with their healthcare provider as it relates to a suspected communicable disease case and any associated exclusions
  • If your child is suspected of having any communicable disease, contact your school nurse
  • Our schools no longer contact trace or require close contacts of an exposure to quarantine regardless of vaccination status
  • Temporary Covid related zoom link (remote learning) accommodations will no longer be offered
  • In order for a student’s isolation period to be considered an excused absence, a note from your healthcare provider must be submitted to the attendance office that affirms the isolation period
  • Students who participate in Extra-Curricular Activities Competition under Policy 2431 shall be required to present written evidence of being free of COVID-19 and eligible to participate in extra-curricular activities competition supplied by the school medical inspector or another qualified physician who has examined the student before the student is permitted to resume participation in any extra-curricular activities competition.
