Dear Parents/Guardians:
The following information is provided to you at this time so you may address them during the summer months PRIOR to the start of school.
PHYSICALS
According to New Jersey State Law (NJAC 6A:16-2.2), a physical examination is required for ALL incoming 9th graders and/or transfer students. The physical is to be within 365 days of the 1st day of school, signed and stamped by your child’s physician. Updated immunization records should be attached with the physical packet as well.
Failure to comply will prevent your child from attending school as per New Jersey State Sanitary Code.
NO ELECTRONIC FORMS WILL BE ACCEPTED, PLEASE MAIL FORMS OR DROP THEM OFF TO THE MAIN OFFICE DURING SUMMER OFFICE HOURS
ADDITIONAL FORMS
If your student has any of the following: allergies, asthma, diabetes, seizure, or needs to take prescription medication during school hours, please return these forms to the Nurse’s Office.
The OTC medication form will be completed on Genesis in the parent portal, under forms for tylenol, advil, and tums. ALL FORMS MUST BE RENEWED ANNUALLY.
All medication brought from home must be dispensed by the School Nurse and must be in the original pharmacy container with the student’s full name, date, and prescription label.
Students may NOT self-administer or carry over-the-counter medication. Only life-threatening medications may be self- administered with doctor’s medication orders (such as asthma inhaler, epi-pens and insulin).
