Athletic Registration

We are pleased to announce that Genesis is now OPEN for sports registration! FamilyID is no longer required moving forward.

All student-athletes that plan to play a sport must have the following items completed before they are allowed to begin participation.

1. They have to be registered via Genesis. 
2. They have to have the hard copy of a sports physical on file with the School Nurse. Click here to download Sports Physical Packet.

Please ensure your child is registered and has their paperwork on file with the school nurse, prior to the season starting for proper clearance.

Forms Due for Sports Participation:

Health Updates: Required for all athletes prior to the start of each season.

Sports Physicals: Valid for 365 days and must be valid at the beginning of the sports season for participation.

Athlete Registration: Athletes will be registering via the Genesis. Please go to the Genesis Parent Portal → Login → Forms → Choose the Appropriate Season Sports Registration Form (Fall, Winter, or Spring).

Health History Update Questionnaire:  Prior to each sport season each student shall provide a health history update questionnaire completed & signed by the student's parent or guardian.  Please go to the Genesis Parent Portal → Login → Forms → Winter Health History Questionnaire.

Please understand that all athletes must have a valid sports physical on file, an updated Health History update on file, be cleared by our District Doctor, and registered on Genesis BEFORE they can participate.
